Key description of the role:
You will work with the Local Authority and local stakeholders to gain referrals for our services and provide a single point of contact for customers wanting to join our services. You will assess customers’ needs and coordinate a program of attendance as part of their personalised journey as well as conducting initial assessments, creating support plans and reviewing progress to ensure their desired outcomes are achieved.
You will be key in building strong and positive relationships with customers, enabling them to make well informed choices and support them to achieve their goals, as well as facilitating strong relationships with families, colleagues and professionals to support the customers’ journey.
The Ideal Candidate:
To succeed in this role you will have the ability to effectively network to build and maintain effective relationships internally and externally, experience of doing this in the context of learning disabilities will be an advantage. You will need strong administration and knowledge of Microsoft packages, excellent customer service and telephone skills and a good knowledge of job market opportunities.
You will possess the ability to support an individual’s progression to achieve goals in an empathetic and caring way whilst being well organised, working calmly under pressure, and managing your own workload and targets.
